At Intercommunity Health & Rehabilitation Center, we provide care in a structured, supportive environment focused on safety, comfort, and continuity of care. Our interdisciplinary team includes licensed nurses, therapists, and support staff who collaborate to address medical, rehabilitative, and daily care needs.

Intercommunity Health & Rehabilitation Center provides comprehensive skilled nursing and rehabilitation services in a supportive, patient-focused environment. Our care programs are designed to meet the clinical needs of individuals recovering from illness, injury, or surgery, with the goal of supporting a safe and timely transition back home whenever possible.

Our facility features a dedicated 30-bed sub-acute care unit, designed exclusively for patients who require a higher level of nursing and respiratory support. This unit serves individuals needing specialized services such as tracheostomy care, cool-aerosol therapy, ventilator support, IV therapy, and advanced wound care, delivered under physician direction and evidence-based clinical protocols.

Intercommunity’s experienced interdisciplinary team works with patients recovering from a broad range of medical and rehabilitation conditions, including orthopedic, neurological, cardiopulmonary, and post-surgical events. Care plans are individualized and adjusted as patient needs evolve, with a strong emphasis on functional recovery, clinical stability, and discharge planning.

Admissions are accepted seven days a week, and referrals are welcome. We encourage patients, families, and care partners to schedule a visit to learn more about our services and care environment.

Our services are designed to support individuals recovering from orthopedic, neurological, cardiopulmonary, and post-surgical conditions. Intercommunity Health & Rehab Center provides skilled nursing and rehabilitation care in a collaborative environment focused on clinical stability, recovery, and discharge planning.

Our interdisciplinary team works together to deliver appropriate care based on each patient’s medical needs and physician direction.

Nursing and Support Services

Providing 24-hour skilled nursing care to support residents’ medical needs, safety, and daily care requirements. Our licensed nursing staff closely monitors patient conditions and coordinates care to support recovery and ongoing treatment.

Rehabilitation Therapies

Short-term rehabilitation services designed to help patients improve strength, mobility, and functional abilities as part of a structured recovery plan. Therapy services are coordinated with nursing care to support progress toward discharge goals.

Sub-Acute Care

Specialized medical support for patients requiring a higher level of skilled nursing and respiratory care.
