Higher-Acuity Skilled Nursing in a Supportive Setting

Sub-Acute & Specialized Care at Intercommunity Health & Rehab Center is designed for individuals who require a higher level of skilled nursing and clinical monitoring than traditional long-term care, but who do not require acute inpatient hospitalization.

Our sub-acute services support patients transitioning from the hospital who need short-term, physician-directed care in a skilled nursing environment focused on stabilization, recovery, and safe transition planning.

Our Sub-Acute Care Program

Intercommunity Health & Rehab Center offers a dedicated 30-bed sub-acute care unit staffed by experienced licensed nurses and interdisciplinary professionals.

Care is tailored to each patient’s clinical needs and delivered under physician orders, with close monitoring to support medical stability and recovery.

Specialized Clinical Services May Include

Sub-acute services are provided as medically appropriate and may include:

  • Tracheostomy care and management
  • Ventilator support and respiratory services
  • Cool aerosol therapy
  • Complex medical monitoring
  • IV therapy and medication administration
  • Advanced wound care
  • Pain management
  • Coordination with rehabilitation therapies as ordered

All services are provided based on physician direction and individual care plans.

Who May Benefit from Sub-Acute Care

Sub-acute care may be appropriate for individuals who:

  • Are transitioning from an acute hospital stay
  • Require short-term, higher-acuity skilled nursing support
  • Need specialized respiratory or medical services
  • Are recovering from complex medical conditions or procedures
  • Require close clinical oversight prior to returning home or transitioning to another level of care
